STUDENT ANNUAL CONSENT AND NOTIFICATION FORM (GF068)

The Annual Consent and Notification Form (GF068) was sent home with all students on Tuesday September 3, 2024. This form includes multiple links and requires annual review and completion by parents/guardians - therefore, a digital copy is included as an attachment below. All parents/guardians are asked to review the form (GF068), including all the links, and return a signed copy to your child's homeroom teacher by Monday September 9, 2024.  Please contact the school if you have any questions.
