To ensure the health and safety of our students, it is imperative that our school is aware of any medical conditions in order to be prepared in the event of an emergency.  Kindly advise the main office if your child has any medical needs.   Specific documentation ("GF514") is needed for the following conditions:  Diabetes, Epilepsy or other seizure disorder, Asthma or Anaphylaxis.  Please see the links below and complete the appropriate form(s).  

NOTE:  Completion of any GF514 form(s) is only needed for those who have not yet previously submitted it to the St. Marcellinus school team, or if changes need to be made to a previously submitted form.​

If medications are required for administration and storage, please complete the additional form:  GF035 - Authorization for Storage and Administration of Medications.pdf.  The GF035 form must be completed and re-submitted to the school on a yearly basis. It does not require a physician's signature if there are no changes to your child's medication or medical condition.
