Message from Dufferein Peel Catholic District Board 

Please complete this form if you wish to request a change in learning environment for your child/children in Grades 9-12 for Quadmester 2, beginning on November 13, 2020 (for example: to switch from in-person learning cohort at school in the morning to full-time remote, online learning or vice versa). If you have more than one child in a Dufferin-Peel Catholic District School Board (DPCDSB) school, please complete a separate form for each child.
