- Government Issued Photo Identification (License, Passport, etc.).
- Provide an email address of requesting institution.
Note: All digital copies must be sent directly from Ascension to requesting institution. We are not authorized to give students digital copies. For hard copies please provide mailing address. Hard copies can take up to 2 weeks to be processed.
